Case presentation Over a three-month period, a 45-year-old girl noted progressive darkening of a longstanding mole, measuring 6 mm in diameter, on the dorsum of her right foot . Dermoscopy demonstrated an asymmetrical mole with an irregular pigment network with regional variation in intensity. A darkly pigmented branched element with globules and dots was at the heart of the mole . Excision biopsy demonstrated nests of atypical melanocytes within the skin and upper dermis, that have been obscured by a marked lymphocytic response.Correlation analysis between demographic variables and humoral response uncovered that alcohol intake was negatively correlated with the responses of the main classes IgM, IgG and IgA, along with IgG4. However, presence of specific IgG4 positively correlated with the fact of being treated with the typical therapy and the standard of necro-inflammatory activity. Additionally, the necro-inflammatory activity, correlated with IgA also, while fibrosis didn’t. These results confirm that while the cellular immune response can be weak and narrow, a vigorous and broad humoral response is present in chronic HCV disease. Especially, the association of particular IgA response to necro-inflammatory activity paves the best way to further research to verify its usefulness as a noninvasive, easy-to-measure marker of histological activity in chronic HCV infection.
